Each Grace client receives a thorough, initial assessment conducted by one of our experienced Registered Nurses. At this time, the nurse evaluates the client’s medical condition(s), home environment, personality, and specific health needs. Using this information, we carefully create and institute an individualized plan of care that we believe will improve their health, maintain their independence as safely as possible, and enhance their quality of life.

Scope of Services

Skilled Nursing:

• Vital signs
• Wound care
• Diabetic teaching and monitoring
• In-home blood draws and injections
• Catheter care
• Tube feeding
• Medication set-up, administration, and management
• Head-to-toe assessments
• Teaching on medical conditions and medications

Home Health Aide:

Home health aide services are covered by Medicare if needed as supportive services for skilled nursing care.
• Bathing
• Dressing
• Light meal preparation
• Light housekeeping
• Laundry

Therapy:

• Physical therapy: The goal of physical therapy is helping you gain or maintain mobility. Services include: evaluation, gait training, transfer training, home exercise program, therapeutic exercise, and more.
• Occupational therapy: Designed to help you regain your independence by performing daily activities by yourself. You may learn new ways to eat, put on your clothes, or comb your hair.
• Speech therapy: After a stroke or accident, speech therapy can help you regain and strengthen your speech skills through exercise and training.

Medical Social Services:

Social work services are covered by Medicare when given under the direction of a doctor to help you with social and emotional concerns related to your illness. This might include counseling or help finding resources in your community.
