What is home health care?

Home health care is an assistance service involving a full range of medical care. Care may include nursing, therapy, and assistance with personal needs. Physicians may order home care after hospitalization or in lieu of hospitalization when skilled care may be accomplished safely in the home. Grace Home Health, in collaboration with the client’s physician, will develop a specific Plan of Care tailor-made to meet the individual needs of the client. This Plan of Care will be monitored regularly and any changes in condition will be communicated immediately to your physician.

Who is eligible for home care with Medicare coverage?

  1. You must be under the care of a doctor, and you must be getting services under a plan of care established and reviewed regularly by a doctor.
  2. You must need one or more of the following: skilled nursing care, physical therapy, occupational therapy, or speech therapy.
  3. The home care agency caring for you must be Medicare-certified.
  4. You must be homebound, which means:
    • Leaving your home isn’t recommended because of your condition.
    • Your condition keeps you from leaving home without help (such as using wheelchair or walker, needing special transportation, or getting help from another person).
    • Leaving home takes considerable and taxing effort.
    • When you leave home, it must be to get medical care, or for short, infrequent non-medical reasons such as the grocery store, the doctor’s office, church, beauty salons, and short visits to other locations.

How much will I have to pay for home care if I have Medicare?

Medicare pays for 100% of home care services. The patient will not be billed. If a patient only has private insurance, a patient might be billed a portion of the fee not covered by the insurance plan. You will be notified if there is a portion that will not be covered by your insurance or Medicare upon admission.

How can I pay for home care services if I don’t have Medicare?

Private insurance companies may cover a range of home care services; however, these vary in relation to each insurance carrier. Services can also be paid for out-of-pocket by the client through our Personal Assistance Services (PAS) program. In addition to Medicare, Grace Home Health also accepts Humana and Secure Horizons.

What does a Home Health Aide do?

The primary function of a home health aide is to perform personal care, such as bathing, dressing, grooming, hair care, and oral hygiene. Once the personal care is complete, the aide may also perform other duties for the patient, such as tidying the room, changing bed linens, or making a snack.

How long can the nurses and therapists come to see me?

As long as a physician orders home care services that he/she deems medically necessary.

How do I sign up for services with Grace Home Health?

Your physician will call or fax us an order, or prescription, for home care services that identifies which services are indicated. We will have the admitting clinician call and set up an appointment time that is agreeable to perform your admission. You may also contact us directly and we can coordinate services with your doctor.
